SOSU ASI Availability Form

All ASI students who are currently enrolled in a flight class

MUST fill out the Availability Form before they can

begin flying each semester.

Click on this link, fill out the form and then print it or email to Professor Jacox (gjacox@sosu.edu)

This form is in Microsoft Word format. If your PC does not have MS Word,

you may use any lab computer on campus or at Eaker Field Operations Building.

You must return the form to Professor Jacox before

you will be assigned a flight instructor.
